Intake forms
Before your first visit
For new patients we run through an intake tailored to the program you are coming in for. Ketamine, neurological, regenerative, weight loss, and osteoarthritis all have dedicated forms. The fastest path is to call ahead. Our team will email the exact form set for your case.
Stroke and TBI lab order
The signed self-pay laboratory order form Dr. Cruz uses for new neurological patients. Includes the CMP, CBC, lipid, thyroid, vitamin D, magnesium RBC, homocysteine, and tick-panel options.
Ketamine intake
Mental health history, medication trials, and risk screening used before your first IV ketamine consult. Sent by our team once the consult is scheduled.
Neurological intake
For stroke, TBI, Parkinson's, and other neurological cases. Focuses on imaging history and functional status.
Osteoarthritis and regenerative intake
Used before PRP, hyaluronic gel, Prolozone, and personal stem cell therapy consults. Joint-focused history.
Weight loss intake
Metabolic history, prior weight-loss attempts, and medication tolerance. Required for HCG and Semaglutide programs.

Questionnaires we use in clinic
Below are the assessment tools we commonly use. The SF-36 is sent on request before neurological and regenerative consults so we have a measurable baseline. Other screens are administered in clinic.
SF-36 Health Survey
A validated 36-question survey covering physical function, role limitations, pain, general health, vitality, and mental health. Used in our neurological and regenerative programs to track change over time.
Patient Depression Screening (PHQ-style)
Brief depression screening administered at every ketamine and mental health intake to track response across an infusion series.
PTSD screening tool
Standard PTSD screening used in ketamine and mental health evaluations, with appropriate clinical follow-up.
Anxiety screening tool
Short-form anxiety screening used in mental health and integrative consults.
